Same errors in my site. I think it`s problem with newer module versions (like CTools, Field group).

I checked files and i found that "formflow_attach_groups" is called before "field_group_attach_groups" so this is the main problem (I think :).
Someone with working version of formflow could check this, please :>

I think, I have answer. Do this query in your DB:
SELECT * FROM `system` WHERE filename = "sites/all/modules/field_group/field_group.module"

Check weight value. Then do another query:
UPDATE `system` SET weight = 2 WHERE filename = "sites/all/modules/formflow/formflow.module"
I set "2", because first query gave me "1", so set "first_query_weight_value + 1".

Clear all caches and check formflow is working now and give some info.

Attached is a patch that updates the module weight so that its greater then the field_group module. Resolves the issues for me.

This should be resolved in the latest dev code. Not by altering the weight, but by fixing some other problems that caused this.
