is it possible to create multi column forms using FB? a la Panels? if not, is it possible to integrate FB with either panels or Display Suite? is any such integration planned? future feature?

Form Builder does not plan on incorporating layout into the functionality of this module. I would suggest looking into Display Suite and http://drupal.org/project/arrange_fields, both of which work with Form Builder, albeit there is no direct integration, they don't interfere with each other.
