Many sites are still using flag-7.x-2.x (in my case I use an Open Atrium 2 distribution, which relies on this).
Since it seems mostly to be a matter of tables names and layout (at a first sight), it should not be difficult to make this compatible with flag 2.x.

Nope, different APIs in flags 3.x and 2.x so not so simple. You might try https://www.drupal.org/project/flag_limit which should be compatible with 2.x version of flags. This module was started since there wasn't much momentum in that issue queue, though.
