I've tried this with 6.11 and 6.x-1.x-dev with the same results. When the module is enabled a blank menu item is created under the user name. Looking at the source it looks like its:

    Looks like I forgot 'type' => MENU_CALLBACK, and MENU_NORMAL_ITEM was being used as default. Download the latest -dev, clear all cache, and if that doesn't fix the problem you might need to go to /admin/build/menu-customize/navigation and click 'reset' on the empty fillpdf-created menu item.
