Hi Tyler

I'm able to load only one pdf file. If I try to add another pdf it is throwing the following error

user warning: Duplicate entry '0' for key 1 query: INSERT INTO fillpdf_forms (fid, title, url) VALUES(0, 'Test PDF2', 'http://example.com/default/files/test.pdf') in /home/user/example.com/sites/all/modules/fillpdf/fillpdf.module on line 21

I think i remember fixing that a long time ago. At least, I don't experience it anymore. tell me if it happens again.
