As reported on #578354: Taxonomy is erased when adding item to new or existing node:

If a node is edited to add an attached file, regardless of whether it was previously created or not, the taxonomy is cleared and has to be reset. Something about that call to "add another item" wipes it out.

I'm experiencing this behaviour as well. (Issue duplicated because that one was pointedly ignored for bundling issues together.)

I have never been able to reproduce this problem or received a confirming bug report. Please reopen if this problem still exists or if the problem can be reproduced from a clean installation.

I'm having this problem with a relatively clean build.
When I upload a file using filefield the taxonomy is reset.

Played around with this and figured out what my problem was.

I had setup a template and defined another version of jquery in the head which was conflicting with the filefield module when it uploads.

A good way to test this is to check whether the page reloads when you click upload. If it does you probably have a similar issue.
