see screenshot from just now using Mac Chrome browser

this issue has appeared more than once after a fix was requested; might be worth adding a test, especially since it probably does not appear for people who have entered a password in the past (I assume it may save that status)

it also does not appear to be fully activated, so I wonder if only a partial effort to remove it occurred.

this looks more serious than we'd originally thought - it's appearing relatively frequently and is likely to deter new users.
