Decisions content type does not appear

I've installed a brand new instance of Drupal and am trying to install the Decisions module.

I've gone to Administer › Site building and checked "Decisions" (see attached screenshot decisions_mod.jpg).
I've set permissions on the Roles who can create/edit/vote on Decisions (see attached screenshot permissions.jpg).

When I go to "Create Content" the Decisions content type does not appear (see attached screenshot createcontent.jpg).

I had this working in the last release, before I wiped my Drupal instance and re-installed it.

How can I get the content type to appear??
