I created an event set to repeat every 2 weeks for approximately 1 year, and added the advanced options of Any Month, Any Day of Month, First Thursday and Third Thursday. The every 2 weeks part works great but you can see it's ignoring the advanced options of only the first and third Thursdays:

* April 7, 2011 - 7:00pm
* April 21, 2011 - 7:00pm
* May 5, 2011 - 7:00pm
* May 19, 2011 - 7:00pm
* June 2, 2011 - 7:00pm
* June 16, 2011 - 7:00pm
* June 30, 2011 - 7:00pm
* July 14, 2011 - 7:00pm
* July 28, 2011 - 7:00pm
* August 11, 2011 - 7:00pm
* August 25, 2011 - 7:00pm
* September 8, 2011 - 7:00pm
* September 22, 2011 - 7:00pm
* October 6, 2011 - 7:00pm
* October 20, 2011 - 7:00pm
* November 3, 2011 - 7:00pm
* November 17, 2011 - 7:00pm
* December 1, 2011 - 7:00pm
* December 15, 2011 - 7:00pm
* December 29, 2011 - 7:00pm
* January 12, 2012 - 7:00pm
* January 26, 2012 - 7:00pm
* February 9, 2012 - 7:00pm
* February 23, 2012 - 7:00pm
* March 8, 2012 - 7:00pm
* March 22, 2012 - 7:00pm
* April 5, 2012 - 7:00pm
* April 19, 2012 - 7:00pm

Because there are 5 Thursdays in June, the repeat goes off the rails at that point.

I'm also encountering this. I'm pulling a copy of the site down to debug, I'll post whatever I find. Edit: Once I cleared the cache, I'm now getting Fatal error: Maximum execution time of 60 seconds exceeded in .../sites/all/modules/date/date_repeat/date_repeat_calc.inc on line 466
whenever I try to use the regular date repeat, without advanced settings.

Unfortunately the Drupal 6 version of the Date module is no longer supported. That said, we appreciate that you took time to work on this issue. Should this request still be relevant for Drupal 7 please feel free to reopen it. Thank you.
