Thanks for this module, it is really great.
I found a bug, that when the date field containts some value, it is show altghought it's control field is off
other fields (text, radios) are correctly hidden.

if date field is blank, then it is hidden too.

I tested it in both latest 1.x. and 2x conditional field module, and with latest CCK dev module and latest date dev module.
