Contact information on checkout missing when customer is logged in.

My guest checkout order information page works great: Contact Info, Payment Info, Shipping Info forms, and Order Summary are all there. But when I'm logged in as a returning customer the Contact Info goes missing. The problem is that my form won't validate. I have UPS shipping and it needs all the info including the Contact Info filled out. I've checked developer tools and it's simply not there. How can I fix this? Please see the images attached for guest (working) and returning (not working) customers on the checkout order information page. Any help asap would be great. Thank you!
