If I'd use memcache-session mechanism instead of the standard DB, than no CAPTCHA image is displayed - just empty image. There is no error message as well. I'm using 6.8 Drupal installation with the last memcache module available.

Yesterday, I released a first beta of CAPTCHA 6.x-2.x, which should solve a lot of problems with the 6.x-1.x branch (related to sessions and cookies).
Can you try if this version solves your problem?
