I very much like CAPTCHA, the latest version works great in Firefox, IE6, IE 7 but Opera can simply by pass it all together. To the point that I can't say it's safe to use.
What would be nice is a way check if it's working, and to gracefully downgrade to a maths question or something.

Can you describe what precisely goes wrong? On which forms, which settings, which users, ....?

If it's on a login form, the problem is probably described in http://drupal.org/node/161584 and solved by http://drupal.org/cvs?commit=80392

Ok, On moving from my dev site to the main site, the Captcha is working OK now. Not 100% sure what it could have been. But it's working now.
