when I upgraded from 1.3 to 1.4 and then clear the entire cache.

I have narrowed it down to toolbar suppression function (admin_select_toolbar_suppress())

I understand that is is probably a permissions based thing but some info on debugging this would be great.

Was in fact a bug in the latest code, sorry. Fixed in 7.x-1.x with http://drupalcode.org/project/admin_select.git/commit/60d057e and will be included in the next release.
