When I create a new image ad.... upload ad image, then attempt to save the new ad. Save fails and "unable to locate image ." error message appears.

I then have to upload the ad image again, flag the first for deletion, and resave. Then the image saves and everything seems to be fine.

Did you get the fixed version of the code, as fixed here:
#436234: CCK nodes do not save on initial creation when ad_channel or ad_weight_probability is enabled

It seems very likely that this is a result of the same bug.
