It sure would be swell if this module had the ability to overwrite a previously uploaded file instead of making a duplicate.

I agree. Here's a patch, rolled from 6.x-1.4

It adds a global "replace" setting. Just go to admin/settings/filefield_paths.

You might want to re-roll the patch to remove the 1st part of the path so that it can be applied with patch -p0. otherwise someone (like me) who attempts to apply the patch will not succeed. This is especially true for those that attempt to pull in patches to Drush Make makefiles.
