Problem with saving forms: only two fields are saved occasionally

Hey guys,

I'm having an issue I believe is stemming from CCK. My current Drupal version is 6.14 (I've yet to update it to 6.15). The following CCK modules are being used:

- Content Copy 6.x-2.6 (don't think it's relevant to the problem, but just in case, I'm listing it here)
- Content Templates 6.x-1.1
- Fieldgroup 6.x-2.6
- FileField 6.x-3.2
- ImageField 6.x-3.2
- Node Reference 6.x-3.2
- Number 6.x-2.6
- Option Widgets 6.x-2.6
- Text 6.x-2.6

There are actually two problems I'm encountering:

The first involves saving the form. After inputting data into the various fields we have created, sometimes, for some inexplicable reason, the form saved would only contain two inputted data. I know this is a bit confusing, so please refer to the attached Capture.PNG.

Under Useable Items, you can see that various data have been entered after clicking on "Add another item" multiple times. Occasionally - and I've no idea how to replicate this problem - after saving the form, what were displayed were only two items, instead of the 10+ things listed in the attached image.

My second problem resembles this issue greatly: #406050: CCK Add Another Item causes fields to disappear. If it's a caching problem, I guess it makes sense why I may sometimes experience the aforementioned first problem? But I don't seem to be able to find any fix to it.
