There are 2 notices in the flags page /admin/structure/flags after installing Flags and Conditional Fields module.

Notice: Undefined offset: 4 in _menu_translate() (line 787 of /drupal root/includes/menu.inc).
Notice: Undefined offset: 4 in _menu_translate() (line 793 of /drupal root/includes/menu.inc).

Steps to replicate this are as followng:

- Use Drupal 7.41.
- Install 2 modules: Flag module and Conditional Fields module.
- Create a flag at least.
- Open flags page (/admin/structure/flags).

Here is a patch.

Only one line should be changed. Seeing the log, this line is changed in the commit 43b52bc1c1e6f7368d54f6dc223809c6694131cf in https://www.drupal.org/node/2397421 . I by myself confirmed that by this change there isn't any regression bug related with the issue. But I hope someone else would confirm it. Thank you.
