Admin menu integration never quite worked. Or maybe it once did and then didn't! ;)

Anyway, I'm going to fix it for admin_menu RC 4 before moving on to updating it for RC 5 at #2397047: Update Flag module to support new hook_admin_menu_map() changes in Admin Menu 7.x-3.0-rc5 onwards.

Two problems here:

- the path for each flag doesn't get a {menu_links} table entry, because hook_menu() sets it to be a callback rather than a menu item. This makes no difference at all to the core menu, but if it's not in the {menu_links} table, admin_menu doesn't see it, and so when we declare our flag items with admin/structure/flags/manage/% they don't get picked up.
- same sort of problem with the fields: the parent needs to be set to something that admin_menu picks up.
