I've added a patch that adds checkbox 'Output label as HTML' to fieldset settings form.
If you check this checkbox, label will be outputted with HTML tags.

The 3 year old patch in #2 to field_group.module applied cleanly to the latest field_group 7.x-1.x-dev, but still needs review (if still applicable).
