I have made sure to go to Permissions and uncheck the options for "Bypass maxlength setting" for the administration role. Despite this, the counter does not appear for the admin.

I have tried both 7.x-3.2 and 7.x-3x Dev, but it's the same issue.

I have tried cleaning the cache. I am using the default Seven admin theme.

confirmed here (plain single line text field from core). Even the limit input field is missing at the field edit form. Test by yourself here: https://simplytest.me/project/maxlength

See maxlength.module at Line ~164:

// Add "$conf['maxlength_always_for_uid1'] = TRUE;" to settings.php to activate
// for user #1.

If you don't want to edit your settings.php you can run a simple drush command:
drush vset maxlength_always_for_uid1 1

Not nice, there should be a setting for this.

Should be documented on the project-page and/or in the README.txt
While testing this module thought it didn't work at all.

#2 is doing the trick yeah.
