Problem/Motivation

Having a plugin implementation to alter field types in the same module doesn't make sense. Core does this because EntityReferenceItem is in core/lib and ConfigurableEntityReferenceItem is in the modules/entity_reference.

Proposed resolution

Merge EntityReferenceRevisionsItem & ConfigurableEntityReferenceRevisionsItem

I still think we should do this together with #2456281: Extend from EntityReferenceItem and remove redundant code.
