If you don't have a formatter with the internal name "default", then no formatter at all will be used. See this issue as an example and this comment for details.

I *think* I remember some discussion about the necessity for field modules to provide a 'default' formatter. This being said, this is not really documented anywhere I can see. Not so sure right now.

Well, before deciding which direction to go (i.e. either providing a patch for CCK that selects the first available formatter as the default, or adding a "default" formatter and writing the documentation), I'd love to read that discussion. Google doesn't get me anything useful though.
