Add a quiz. While adding a quiz check "Mark Doubtful" option in "Taking Options" tab.
Take the quiz.
Open the Console tab of firebug.
1) A 404 Not Found - /sites/all/modules/quiz/theme/quiz_take.js?mk9e7d" error is thrown.
Because the file quiz_take.js doesn't exists.
2) Also while taking quiz a "doubtful" check box & "Clear button" will also appear below the question. But that clear button doesn't work and it does not clear the doubtful checkbox if checked.

Please find the quiz_take.js file which contains the code to clear doubtful checkbox (if checkbox).
