Requesting new password doesnt show password fields at same page as save button

I'm using the Password change tab 7.x-1.3.
Tried to request a new password and end up with a page showing me a "login" button. The next page instructs me that I have to choose another password, but it only shows my username and a Save button.
The fields to enter the password are not there but at another page named password (which is there because I enabled the Password change tab.

I just tried to press save without entering a new password, which seems to be accepted but a error is shown: Notice: Undefined property: stdClass::$name in taxonomy_access_field_attach_validate() (line 299 of /home/admin/domains//public_html/sites/all/modules/contrib/taxonomy_access/taxonomy_access.module).
