We added the media_title field to address the lack of ability to give files a custom title. However, since then, file_entity + media makes the file name field editable and therefore meets this need.

Therefore we should remove the media_title field.

TBD: what to do about existing sites? Remove the fields? Set them so they're unlocked and can be manually deleted?
