Bug fix for child links block and additions to README.MD

- Changes default to the right of the "Manage Toolbar"
- Fixes bug in the css file's name in the libraries file.

This is a pretty significant rewrite of the module from the previous alpha release.

NOTE: This may be break backwards compatibility. Re-saving the field formatter settings on the manage display page should resolve any issues.

If no major bugs are reported against this release over the next week or so, then a full release is eminent and I will have security coverage.
