Bug fixes

This is the first release for Bootstrap 4. It now has cards added and amends of col naming and markup.

Few corrections added:
- ' is not converted to single quotes.
- Corrections on warnings: "only variables can be passed by reference"
- Extra space added after bold, italic and other markup syntax
- Allow /commands with no text.
