Bug fixes

Requires database updates - please run after upgrading.

- Adds support for grade levels! To enable, visit the Reading Rating settings on the form display. The grade levels, like the ratings, can also be configured in the settings under /admin/config/content/reading-rating
- Fixes bug where sometimes Reading Rating would not update immediately.
- Reworks code and templates to prepare for future additions.
