Switching to OOP in managing node widget forms. Various bug fixing.

Fixed bug which would have prevented the module from working properly on sites using a database table prefix or other non-basic database configurations.

#722262 - nhepner1 - Fixed missing { } for table og_uid.

fixed fatal error

Closed tickets 698540 and 710462 and while doing it cleaned up the CSS style.
Added theme tagline.

Bug fix: terms which have no term field may cause issues (undefined variable $fields)

Upgrading from RC3 is a must due a syntax error in the code. Big mistake of myself, sorry.
