New features

Cleaned up the code and made it into a full release.
Hopefully I'll get around to porting this to D8 soon!

Fixes several bugs.

• In particular, fixes error when first installing the module or updating from an older version.

Also adds new features:

• Role and menu fields are now selection fields (dropdowns), not text fields
• Disallows duplicate pairs
