Updated module to work with Entity Reference fields.

• New release makes better use of AJAX to display/hide controls on the configuration page.
• Revision All now assumes you truly want to revision all current and future content types. As a result "Enable for all Future Content Types" is enabled and hidden if "Revision All" is set.
• Internal variables have been renamed to fit D7 naming conventions. Please run update.php or drush updb to auto convert to the new format.
