Description says "The setting is in the content type form".
I can't find it in admin/content/node-type/[content-type].
Am I looking at the right place?

It should be under "Submission form settings" and is titled "Duplicate submission buttons at top".

I can't see this setting there - do I have to have a specific Util sub module installed eg System Module?

Well, yes, you need to enable the Top Buttons module, which is part of the Util collection, but only in the latest 6.x-3.x-dev release.
