I had an issue that when another module adds a field to $form['account'] that the $form['account'] array is overwritten such that a number of important fields are gone.

a solution for this is to replace array_merge with array_merge_recursive on line 293

Not sure under what conditions to test this, but here is a patch that addresses the above.
