System log says module is both installed and enabled, and Modules list shows it as enabled, shows up in Permissions list, but there's no table for it in the database, and no configuration settings on any of the field admin pages. Never had this happen with any other module.

Unique field does not have separate db tables, it puts its settings in the variable table. You can set it up on the content type's setting page (e.g. admin/structure/types/manage/article).
