Kerberos Authentication Module 7.x-2.2
TFA configured to use TOTP

All accounts other than uid-1 account are configured to use Active Directory for authentication, using the Kerberos module. I can activate TFA for other users using the uid_1 account/password, but these users cannot activate TFA themselves because the "TFA setup" form always errors with "Incorrect password" message. It appears that TFA Basic can only handle local accounts/passwords.

This might actually be the problem I'm currently having! We are using the LDAP module.

None of the maintainers of the module use AD, so the fix here will likely have to come from somebody who is using that combination.
