I had SWF Tools set up and running perfectly, but then needed to reinstall Drupal (upgraded from 6.13 to 6.14) and all the modules I was using, and then encountered this problem.
I DO NOT see the test animation on the SWF Tools status page.
When I navigate to a page with a link to a flv file [swf file="demo2.flv"] I do not see anything - not even the message notifying me that flash content is missing. However, if I delete my temporary internet files and refresh, the player is there, and plays the file just fine. If I navigate away from the page and back, the player disapppears again.

Stuart Greenfield commentedAre you using FlowPlayer3?
Do you get this in all browsers? Or just some?
jasonjason commentedI am using FlowPlayer3 and SWFObject 2 - JavaScript.
This problem is happening for IE 7.0, but not in Firefox 3.5.3

jasonjason commentedI'm sorry - I mis typed, I am not seeing the player, upon navigating away from page and back, in either browser (IE 7, FF 3.5.3)
