I know its not currently part of subscriptions, but my clients would really like to be notified of changes immediately, instead of after each cron run. Can this be added, or if not, can someone suggest how to send the emails immediately or fake cron in order to get it to work?

No, it's technically not possible to implement this correctly.

Just set your server to run the cron job every minute, if that's what you require. There's no harm in doing that (except for lots of watchdog entries).
