I can see the Custom tab in the Spam admin page, but it never got installed since I don't see the Custom filter in the list of modules (which is probably why the spam_custom table never got installed).
I see all other spam/filters directories, but not custom, which is very strange. custom/custom.info looks correct to me, it is at least similar to the url/url.info which does show up in the list of modules.
