Site crash after installation...

I've just installed 7.x-1.0 on Drupal 7.14 and have no problems. There are 55 installations at the moment, but you're the fisrt who reported about this issue, so this is not a regular problem.

What version of Drupal do you use? "Clean drupal 7" you mean that you installed D7, not upgraded from D6 to D7, correct?

Looks you have no {cache_simplemeta} table in the database, but this table is defined in way core modules define cache tables, so it's very strange that there is such problems on installation

Any additional info will be very helpful.

I have exactly the same problem :

PDOException: SQLSTATE[42S02]: Base table or view not found: 1146 Table 'pacificg_pg.semaphore' doesn't exist: SELECT expire, value FROM {semaphore} WHERE name = :name; Array ( [:name] => variable_init ) in lock_may_be_available() (line 167 of /home/pacificg/public_html/includes/lock.inc).

Uncaught exception thrown in shutdown function.

Two crash in 3 days, and all Drupal websites (7.14, 7.12 ) are affected !

Actually you have not the 100% same problem. You have problem with {semaphore} table which is not a part of Simple meta module, it is a part of Drupal core...
