Configure default value for the form fields

Problem/Motivation

Currently there is no possibility to configure default value for the field with an option to hide field or exclude the field from the form.

Proposed resolution

Extend configuration form for the fields by providing 3 more fields: Hidden field, Exclude from form, Default value.

Default value field should support tokens.
