The admin list prints the module title instead of the delta. This is confusing!

Added an actual title column.

This patch does some code fixes to that function too which I know is bad form... but I had to at least put in the colspan :/

- all functions should have a doc header
- don't put empty cells in the header to cover the operation cells, just use colspan
- ...and in fact, put in an 'Operations' header (with a colspan)
- the . operator takes a space each side
- foreach keyword takes a space before the opening bracket
