I am going to number these as what is expected from this description listed here
http://drupal.org/node/1011698
1) This module always for the Referral Sources module and UC Affiliate 2 module to work together, and allow for affiliates (based off of the user role) to be displayed inside of the Referral Sources drop down lists that are displayed on the User Register, Ubercart Content Pane, and Webform forms, to perform the same function as clicking on an affiliate link. It does this by adding a linker table into the database, which associates referralsources with users, and there in, linking them to affiliates. Once the user accesses the system settings form, and activates the module (I have done this as there are a couple of tweaks that they may wish to make before the affiliates are inserted into the database), the affiliates will show up in the drop down list for the forms listed previously. Upon a successful form submit, the affiliate module will be accessed, and the settings by which the site uses to connect a user to an affiliate will be set (cookies are used on my site, and works correctly).

5) Drupal 6

hazaThanks for your submission.
You could use the "coder" module to check the coding standards of your module. I see you use tabs in the files, drupal only use spaces for exemple.
Also, more important, you should learn how to make correct SQL query in drupal.
Always use placeholders when writing SQL query.
See Writing secure code for some exemples. Plus, you need to add brackets {...} around your table name. Drupal will recognize it and add table prefix if needed.

This was just a quick look on the code, i didn't go too much deeper.
