In the config I cannot select the custom button image radio button. Even if I put in a URL it will not let me select it. The rest of the radio buttons are fine.

It looks like it is simply a CSS issue. The div that holds the custom button url field is layered on top of the radio button.

Here is the CSS when I use the inspector:

#printfriendly-config-form .form-item-custom-button-img-url {
    display: inline-block;
    vertical-align: top;
    margin: -40px auto auto auto;
    padding: 0px 0 15px 15px;
}

I was able to click on the radio button by changing the margin-left. For example:

margin: -40px auto auto 1em;
