does the node_notify performance scale well? would it be able to handle thousands of subscribers to a particular node? tens of thousands? does it use cron to fire out the emails?

I don't have any hard numbers on that, I can only speculate. Number of subscriptions is a matter of data store scalability. If your DB can join two tables on primary key fast, you'll be fine. Sending messages is more prone to scalability issues. Each message is sent as soon as comment is made. There is no queue to throttle -- you may get a lot of message to send at the same time and this means that DoS is possible.
