The placeholder image isn't passed to the theme function.

Additionally, if you specify no loader image, the js for it gets added anyway. Very closely related these two so one patch.

This fix wasn't correct, marking CNW to fix it properly for the case when there's no placeholder at all.

In dev this should be fixed, using white.gif when no placeholder is set.
