Help with flag module integration

I'm trying to have flag work for filefield files and I'm having a hard time getting the flag link to output in the right place. I decided to start with imagefield images, since it looks like theming happens per widget and there isn't an overarching them function for all filefield output.

I'm hitting a wall in trying to just add the flag link to ALL imagefield output. I've got it for the filefileld core output, but then with imagecache it doesn't work, I have to theme override the imagecache formatters. From the devel themer module, it looks like there isn't even an overarching theme function for all imagefield output. Do I have that right?

in a way it's good, i could set the flag options to show the link only on certain imagecache presets, but then what about other formatters... i.e thickbox?
