not resizing image

I have a strange problem. I created several accounts to test my site. Profile images seem to be working ok on all but one profile. The profile image on that users profile is double the image cache preset. How could this sort of thing happen? Do I have a setting wronge? I created the presets and set them on the user settings page. Any help would be greatly appreciated.
