This runs in parallel to #1672686: Option to return an empty string from the theme functions instead of a broken image/link if the path can't be repaired. Users should have the option to just remove all broken images from the page without any default image replacement.

Refactored a lot of the current code and replaced the "inject default image into empty CCK fields" with 4 different basic module behaviours:

- don't modify urls/paths by default
- replace *all* broken paths (including empty paths provided by cck)
- replace only broken paths that look like they were once valid, remove other broken paths
- remove *all* broken paths
