Permissioning Problem

I've had form_mail installed for a long time and just received notice that the page does not work and gives users a "Page not found" error when they submit. Upon investigating I found that I have a permissioning issue. When I'm logged in as an Administrator it works great, but if I'm coming in just as a user I get the "Page not found" error.

Any ideas? I'm not sure which file I have a permissioning issue on.
